My congrats go out to Stacey, a Neuroscience Nurse, who is the latest winner of our Editor @ Large contest. She’ll be receiving a $50 gift certificate to Lydia’s Uniforms that she can use to buy stylish new medical scrubs. Here is Stacey’s winning comment:

Nursing was simply a calling for me. At the age of twenty-four, I suffered a herniated disk while at work. I endured nine months of debilitating pain. At the time, I was a young wife and mother to a 2 1/2 year old beautiful daughter. This was to be the happiest time of my life. But the pain I experienced was so severe that I considered ending my life. Then, finally with the benefit of an excellent neurosurgeon my pain was relieved. The nurses that cared for me post-operatively renewed my faith that I could go on and accomplish anything that I set my mind to, which at the time included walking. Finally free from my pain, I decided to become a nurse. I understood the pain that my patients would experience. I am now a Neuroscience Nurse. I see patients with injuries so much worse than what I suffered. I am able to inspire them and give them hope for a life that is worth fighting for, by my example. Being a part of someone’s recovery is a gift that truly never stops.

Unwritten Rules within the Nursing Profession

Like many professions, don’t you think there are many unwritten rules in nursing? Depending on where you work, you have probably been exposed to many of these tacit expectations. Some of these norms are more obvious than others.
 
For instance, beginning with nursing school, most nurses are told that there is the “ideal way of doing things in our profession,” and then, there is the “real way things are done.” I remember hearing this idiom when I first started out as a nursing assistant. It still sounds cynical to me.
 
Since the early days of nursing, there seem to have been several unwritten rules based on convention. When it comes to health and life-and-death situations, it makes sense to have some hard-and-fast rules. However, at times, inflexible “traditions” may interfere with a nurse’s critical thinking. They can also hamper the potential for promoting positive change within a facility.
 
This brings me to a common, unwritten rule among certain groups of nurses: Don’t ask too many questions. Having precepted students and oriented new nurses, I can definitely identify with this notion. Nurses stay busy, and questions (especially complicated ones) can slow us down. Too many questions can interfere with our already hectic workload.
 
One of my favorite responses to an overly inquisitive student or new nurse is to have them briefly tell me what they think the answer is. This approach empowers them to learn to think through their questions before approaching busy nurses for an easy answer. If practical, I encourage novice nurses to research some answers on their own. Of course, this depends on the type of question. For theoretical questions, I try to give them a quick overview and then encourage them to look at relevant literature. On the other hand, when questions are in regard to ongoing duties or imminent procedures, then quick (and accurate) answers are essential. Sometimes, we must interrupt our own work flow in order to assist new nurses with equipment, clinical skills, or locating vital supplies.
 
Some health care facilities seem to have an unwritten rule that nurses must be fast and efficient. While I believe that nurses should be organized and effective, they must also be tuned into the needs of the patient and family members. Patients don’t like to feel rushed, and rushing can lead to errors. Furthermore, it may not be effective to try to rush all of our tasks into one hurried encounter with our patient. Take, for instance, patients with increased intracranial pressure or the unstable patient who may desaturate with too much stimulation. These patients need to have their bedside care doled out in easily manageable pieces.
 
Another unwritten rule in nursing seems to be that a seated or talking nurse is somehow unproductive. Most of our seated time involves charting, which is very much a part of our expected nursing role. And, talking to patients and family members is not necessarily nonproductive. We may be calming our clients, assessing their mental status, or building their trust. This unwritten rule is closely related to the all-too-often “norm” of expecting nurses to skip meals and interrupting them on their way to the rest room.
 
There is a great deal of physical work involved in the routine bedside care that we provide everyday. Hopefully, your facility doesn’t have an unwritten rule that every nurse should “tough it out on his own.” I prefer sharing the load, whenever possible. Many hands make for a lighter load.

Last Night With A Sweet Resident

As a charge nurse working second shift in a nursing home you never know what kind of a surprise you're going to get. I'm going to tell you about a shock I recently had while at work and a loving resident that I will never forget. I came in extra as alot of us nurses do on my night off one Tuesday night. I was down my hall in which I have long term care residents passing HS meds when one of my residents (let's call her Susie just to give her a name, not her real name) said to me, "Nurse, I'm starving". Being the type of person I am I went and thickened her up a coke (which if you've ever thickened soda before you know it's tricky) and got her some goldfish crakers. I got a tray table for Susie and she sat by my medcart in her wheelchair and proceded to eat her little HS snack and then she looked up at me and stated, "These are delicious, so cheesy, and crunchy, and you can't have any!" Mind you Susie has dementia and is always spealing off silly comments such as, "This is crazy", "Where am I" and my personal favorite ( she has 3 daughters) she'd ask, "Does my family know I'm here?" and the correct response would be "Yes, Susie, they all know you're here" and she would be appeased. So I laugh at her comment and say " Susie, I would never want any of your snack, I brought those just for you." and she smiled at me. Well the night went on and the CNAs took her to bed and I went home and then came in the next night. Things weren't the same with Susie. She was vomiting and her abdomen was firm and distended and I sent her out to the ER. All the while she's saying, " I'm fine, I'm not sick" and " I don't need to go to the hospital." Her daughter that lives nearby had been called and wanted her to go so she was sent to the ER. The next day I come into work to the most horrid news. My Susie was coming back into the Hospice unit with 3-5 days to live due to advanced cancer that had spread throughout her abdomen. I was so stunned I had to sit down. I went and saw Susie in the Hospice Unit and her three lovely daughters all hugged me and thanked me for all the loving care I had given their mother and we shared all our funny Susie stories. She died within 5 days and I truly miss my funny Susie. But it's a lesson on life that no matter what, don't take your time with friends and loved ones for granted because they can be eating crackers with you one day and terminal the next. Live each and every day to it's fullest.

Congrats to Our Latest Contest Winner

Today I want to send my congrats to Beverly, a registered nurse with certification in psychiatric/mental health nursing. Beverly is the latest winner of our monthly contest and she won by making us laugh with this comment:

An elderly patient was admitted to a geropsychiatric unit. During her assessment, the nurse noted that she was allergic to nuts and indicated this on her allergy band. During visiting hours, her son approached the nurse angrily, "What do you mean labeling my mother 'Nuts'"?

Daddy, You Were My First Patient

I was 16 when my father was diagnosed with cancer. I remember the last few days of his life. Hospice had been called in. I watched the nurses take care of him. They gave him his medicine and bathed him. But I wanted to be the one to take care of him. I wanted to make sure he had everything he needed. I could not give him medicines then but one thing I could do was make sure he knew that I would be there to hold the pan when he needed to throw up, I would be the one to put cold cloths on his head and I would make sure he knew I was there, that I loved him. From that moment on I knew I could make a difference and serve others w/my heart...
